Nurse Case Manager- Our nurse care managers are experienced in providing palliative care to individuals with advanced diseases. By coordinating with your physicians, your nurse case manager will help you achieve your treatment goals. Expertise from our palliative physicians or nurse practitioners, along with your nurse case manager can assure your pain and symptoms will be addressed and relieved to the highest degree possible.

Physician- A palliative physician or nurse practitioner may visit you. Our provider is a specialist of symptom management for patients with serious illnesses. They're board certified by the American College of Medical Specialties. Their focus is to help relieve pain and suffering, by using evidence-based treatment plans. Care is provided along with your other physicians to reduce symptoms, increase clinical coordination to continue your ongoing treatments.

Medical Social Worker- Assist you and your family with challenges you may be facing. they are experts in the review and application of services and resources you may need. Our focus is comfort, support, resources and education of your unique experience. Ex. IHSS, food, housing, transportation, supplies, emotional guidance, etc...

Chaplain- Our non-denominational chaplain will be available to support you and your family.
